Who is eligible
All individuals residing in the United States who were sent a notice of the data incident indicating their private information may have been impacted.
How you get paid
Class members may choose either Cash Payment A — reimbursement of up to $5,000 for reasonable documented losses from fraud or identity theft — or Cash Payment B, an alternate cash payment estimated at $75. Either option may be combined with one year of medical data monitoring. Cash payments are subject to pro rata adjustment.
Proof required
Cash Payment A requires reasonable third-party documentation such as telephone records, correspondence, or receipts; personal certifications, declarations, or affidavits alone are insufficient. Cash Payment B and medical data monitoring require no documentation.
