How do you get paid?

Category 1 — reimbursement of documented out-of-pocket losses up to $3,500, plus compensation for lost time (up to four hours at $20 per hour, maximum $80); or Category 2 — a one-time alternative cash payment of $45 with no proof required. All Class Members may also enroll in three years of CyEx Financial Shield Complete credit monitoring.

The $45 alternative cash payment requires no documentation. For documented losses, proof such as bank statements or receipts showing the expense resulted from the incident; lost-time claims require a brief description of the time spent.
