People who were mailed notice that their protected health information may have been affected by the October 2024 DATS data breach.

How do you get paid?

Up to $5,000 for documented out-of-pocket expenses, or an alternate flat cash payment (adjusted pro rata). Claimants may also elect one year of CyEx medical identity-theft monitoring.

What proof do you need?

The flat cash payment requires no loss documentation. For documented losses, documentation of the out-of-pocket expense. Filing requires the claim details from your mailed notice.
