Family Medicine Centers (FMC Services) Data Breach Settlement
FMC Services, LLC settled allegations that an unauthorized third party accessed and acquired information stored on its computer systems on or about July 26, 2022, allegedly compromising patients’ personally identifiable and protected health information. FMC Services, LLC denies any wrongdoing and the court made no finding of liability.
A $2,150,000 settlement over a July 26, 2022 data incident in which an unauthorized third party accessed FMC Services’ computer systems. If you were sent notice that your Private Information was potentially compromised, you may claim documented-loss reimbursement or a flat alternate cash payment, plus medical data monitoring.

- How you get paid
- Cash Payment A reimburses documented losses up to $5,000.00 per Settlement Class Member, or Cash Payment B is an alternate cash payment estimated at $75.00 (subject to pro rata adjustment). Adult Class Members may also elect Medical Data Monitoring.
- Proof required
- Cash Payment A requires reasonable third-party documentation of losses (e.g., telephone records, correspondence, receipts); Cash Payment B requires only a valid Claim Form electing the alternate payment. Self-prepared declarations alone are not sufficient documentation.
