Hillcrest Convalescent Center, Inc. settled allegations that a June 2024 cyberattack on its computer systems may have resulted in the unauthorized access to and/or acquisition of certain Private Information, including names, Social Security numbers, financial account information, driver’s license numbers, and medical and health-insurance information. Hillcrest Convalescent Center, Inc. denies any wrongdoing and the court made no finding of liability.

A settlement over a June 2024 cyberattack on Hillcrest Convalescent Center’s computer systems that may have exposed personal, financial, and medical information. If your Private Information was compromised, you may be eligible for Settlement benefits and a cash payment by filing a claim.
