People sent notice may be able to claim cash payments or reimbursement of documented losses, plus medical identity monitoring.

All individuals whose Private Information was implicated in the October 2023 data incident, including everyone to whom the defendant sent an individual notification letter.

How do you get paid?

A one-time $80.00 Alternative Cash Payment (no proof required); or reimbursement of documented ordinary out-of-pocket expenses up to $550.00, documented extraordinary losses from fraud or identity theft up to $5,500.00, and lost time up to $125.00. All class members may also enroll in three years of CyEx Medical Shield Complete.

What proof do you need?

The Alternative Cash Payment requires no documentation. Reimbursement claims require proof such as receipts showing the expense resulted from the incident; self-made notes alone are not sufficient. Extraordinary-loss claims also require showing the loss was likely caused by the incident and mitigation was attempted.
