- From a $3,300,000 non-reversionary fund: reimbursement of documented out-of-pocket losses up to $5,000, a pro rata cash fund payment, and two years of Medical Shield Complete medical identity-theft protection and monitoring through CyEx.
- Proof required
- The out-of-pocket reimbursement requires documentation of the claimed losses. The pro rata cash payment and the monitoring benefit require no proof.
