How you get paid
Cash Payment A — reimbursement of documented out-of-pocket losses up to $1,000; or Cash Payment B — a one-time pro rata share of a $50,000 settlement fund, divided equally among valid claimants. All Settlement Class Members may also enroll in one year of CyEx Medical Shield Complete credit monitoring.
Proof required
Cash Payment B requires no documentation. For Cash Payment A, proof such as bank statements or receipts showing the expense resulted from the incident.
