Who is eligible

U.S. residents whose private information may have been affected by the October 2024 Onsite Mammography data security incident and who were sent written notice.

How you get paid

Reimbursement of up to $5,000 for unreimbursed costs, plus pro rata cash payments, from a $2,525,000 fund. Class members may also receive credit and medical-data monitoring.

Proof required

The unreimbursed-cost reimbursement requires documentation; the pro rata cash payment and monitoring require no proof.
