- Reimbursement of up to $5,000 for documented out-of-pocket losses incurred between September 17, 2024 and September 29, 2026. All class members may also enroll in two years of CyEx Medical Shield Complete medical data monitoring with $1 million of medical identity theft insurance.
- Proof required
- The documented-loss payment requires reasonable proof such as receipts; personal certifications, declarations, or affidavits do not by themselves constitute reasonable documentation.
