How do you get paid?
Reimbursement of up to $5,000 for documented out-of-pocket losses incurred between September 17, 2024 and September 29, 2026. All class members may also enroll in two years of CyEx Medical Shield Complete medical data monitoring with $1 million of medical identity theft insurance.
What proof do you need?
The documented-loss payment requires reasonable proof such as receipts; personal certifications, declarations, or affidavits do not by themselves constitute reasonable documentation. Filing online requires the Unique ID and PIN from your mailed notice. Medical data monitoring requires no loss documentation.
