Who is eligible

All current and former employees, and their family members, as well as current and former contractors who received notice from PCLS whose personal information may have been impacted in the data incident.

How you get paid

Up to $250 for documented ordinary out-of-pocket losses and up to $4,000 for documented extraordinary losses; or reimbursement for attested lost time at $20 per hour up to 4 hours ($80 maximum); or an alternative cash payment. These cash benefits are subject to a $385,000 aggregate cap and may be reduced pro rata. Three years of credit monitoring and identity restoration services are also available.

Proof required

Ordinary and extraordinary loss claims require third-party documentation (such as receipts); self-prepared documents alone are insufficient. Attested lost time requires an attestation. The alternative cash payment and credit monitoring require no loss documentation.
