The approximately 58,309 individuals who were notified their personal information may have been compromised in the May 2024 Datavant (Ciox Health) data security incident.
How you get paid
Either Cash Payment A — reimbursement of up to $5,000 for documented out-of-pocket losses and time — or Cash Payment B, an alternative pro rata cash payment. Either option can be combined with one year of expanded identity-theft and fraud monitoring.
Proof required
Cash Payment A requires documentation of the claimed out-of-pocket losses. The alternative Cash Payment B and the credit-monitoring option require no proof. Filing uses the Settlement Class Member ID from the mailed notice.
