Individuals who were notified that their personal information was compromised in the May 1, 2023 Deanco Healthcare / Mission Community Hospital data security incident.

How do you get paid?

Reimbursement of up to $5,000 for Documented Losses; a $100 California Statutory Payment for California residents at the time of the incident; and a pro rata residual cash payment. All class members may also enroll in medical monitoring.

What proof do you need?

Documented-loss claims require proof such as receipts or statements; the residual cash payment requires no proof, and the California statutory payment requires attesting California residency.
