Mount Sinai Medical Center of Florida denies any wrongdoing and the court made no finding of liability.

Class Members can claim an estimated $20.00 cash payment, and receive one year of free Medical Data Monitoring without filing. Claim Forms are due September 28, 2026.

- A Cash Payment in the estimated amount of $20.00, which may be subject to a pro rata increase or decrease depending upon the total value of all Valid Claims. Without submitting a Claim Form, Class Members receive an activation code for one year of free Medical Data Monitoring.

- A timely and valid Claim Form. No Claim Form is needed to receive the Medical Data Monitoring activation code, which is located on your Settlement Notice.
