How do you get paid?

The official site states that Settlement Class Members could submit a Claim for a Cash Payment of up to $5,000 upon presentment of documented losses related to the Data Incident, plus either a credit and identity monitoring product or, as an Alternative Cash Payment, an estimated $100 cash payment subject to pro rata increase or decrease.
