The official site states that all Participating Settlement Class Members automatically receive a $10 Voucher and Identity Theft Protection without having to file a claim, and that those who file a Claim Form are eligible for documented out-of-pocket expenses directly caused by the Data Incident, not to exceed $1,250 per Settlement Class Member.
