Summit Medical Group, PLLC settled allegations that during the September 2024 targeted cyberattack on its computer systems, certain files that contained Private Information were accessed, which may have included names, contact information, medical record numbers, treatment information, health insurance information and Social Security numbers. Summit Medical Group, PLLC denies any wrongdoing and the court made no finding of liability.

All living individuals in the United States whose Private Information was potentially compromised in the Data Incident, including all individuals who were mailed notice of the Data Incident from Defendant.

How do you get paid?

Cash Payment A: up to $2,500.00 for actual, documented out-of-pocket losses due to the Data Incident. Cash Payment B: up to three hours of time spent responding to the Data Incident, at $15.00 per hour, for a maximum of $45.00. Any cash payment may be decreased on a pro rata basis if more than $500,000.00 is claimed by eligible Class Members. All Settlement Class Members are also eligible to enroll in two years of CyEx Medical Shield Complete.

What proof do you need?

Cash Payment A requires proof, like receipts, showing how much you spent or lost; personal certifications, declarations, or affidavits alone are not reasonable documentation. For Cash Payment B, you must attest that the time claimed was spent as a result of the Data Incident.
