Class Members may enroll in two years of medical identity monitoring and choose one of two cash payment options: reimbursement of documented losses, or an alternative cash payment that requires no proof.

All individuals residing in the United States whose Private Information may have been compromised or otherwise impacted in the Data Security Incident, including all those who received notice of the breach.

How do you get paid?

Class Members can get back up to $5,000.00 for documented out-of-pocket losses due to the Data Security Incident, or instead claim an Alternative Cash Payment, a one-time pro rata payment expected to be $75.00 that may be larger or smaller depending on the total claims filed. All Class Members are also eligible to enroll in two years of CyEx Medical Shield Complete.

What proof do you need?

Documented Loss claims require proof, like bank statements or receipts, showing how much you spent or lost because of the Data Security Incident. You do not have to provide any proof or explanation to claim the Alternative Cash Payment.
