Valley Oaks Health, Inc. settled allegations that during the June 2023 targeted cyberattack on its computer systems, certain files that contained private information were accessed. Valley Oaks Health, Inc. denies any wrongdoing and the court made no finding of liability.

Who is eligible?

All persons whose Private Information was potentially compromised as a result of the Incident and who are included on the Settlement Class List. The Judge and Court personnel, Valley Oaks and its affiliates, and anyone found guilty of causing the Incident are excluded.

How do you get paid?

Class Members can claim reimbursement of ordinary documented out-of-pocket expenses due to the Incident of up to $500.00, reimbursement of extraordinary losses from identity theft or fraud of up to $5,000.00, and lost time of up to four hours at $20.00 per hour, for a maximum of $80.00 — or, instead of any other payments, a one-time $40.00 alternative cash payment. All Class Members are also eligible to claim two years of CyEx Medical Shield Pro medical monitoring.

What proof do you need?

Loss reimbursements require proof, like receipts, showing how much you spent or lost because of the Incident; lost-time claims require a brief description of how the time was spent; the $40.00 alternative cash payment requires no proof or explanation.
